Community Health Assessment (CHA)
The CHA provides a comprehensive look at the health status, needs, and strengths of Delaware County. This assessment includes insights from local data, public input, and partner organizations to identify the most pressing health challenges facing our communities.


Community Health Improvement Plan (CHIP) 2024–2028
Guided by the findings in the CHA, the CHIP outlines a strategic roadmap for addressing key health priorities over the next five years. This plan brings together stakeholders, sets measurable goals, and drives coordinated action to create a healthier, more equitable Delaware County.
