The Admissions Coordinator is responsible for collecting and disseminating, to appropriate disciplines, the pertinent health information of applicants as they prepare to be admitted to the facility. The Admissions Coordinator works closely with applicant’s families and/or responsible parties, members of the acute care facility d/c planning teams, and community medical professionals to ensure that pertinent medical information has been obtained, to the best of our ability, to provide the Fair Acres staff with a history of the applicants medical and mental health.
Responsibilities:
Initiates morning meeting with Admission Department staff to maintain priority waiting lists. Works closely with admission caseworker(s) to determine appropriateness of prospective admissions and readiness for admission.
Works in conjunction with Admissions Case Manager to determine bed availability.
Process requests for room and floor changes, hospital returns when requested.
Reviews, scans, and enters medical/psychiatric information into the preadmission database. Sends information as needed to Medical Director for review.
Contacts social workers from assisted living facilities, nursing homes and hospitals for additional information and expected discharge dates.
Obtains nursing history information for pending admissions from family members, nursing personnel, and community physicians.
Obtains authorization/precertification number from social workers for Medicare/Medicaid HMO’s from discharging hospitals, NH, and home, if applicable.
Provides admission representative with notification sheet and when applicable, provides the PASRR, 408 and Target letter to fax to Utilization Management Review Team within 24 hours of admission.
Completes PASRR (Preadmission Screening Resident Review) for each admission.
Enters date and time of admission, assigns unit and room number, and time of family to arrive to FAGC for day of admission in the admissions database (notification screen.)
Prepares and reviews target list for Utilization Management Review Team quarterly review, providing social service with copy. Reviews list with UMRT Coordinator and obtains missing PASRRS as needed. Attends entrance and exit quarterly UMRT review conferences.
Initiates Psych Reviews with Medical Management and Psychiatry.
Provides pertinent information to Infection Control and Performance Improvement, Restraint Reduction, Wound Nurse, and Nursing Administration, when needed, as it pertains to new admissions.
Contacts physician’s office for completion of MA51, as needed, for pending new admissions.
Contacts physician’s office to obtain vaccination record and other information, as needed, for pending new admissions.
Review and process referrals from Allscripts and Curaspan for viable admissions. Access MVP, Navinet and Promise for verification as needed.
Notifies Nursing Administration information regarding pending admission, when applicable.
Requirements:
Graduate of an Accredited School of Registered Nursing. BSN preferred.
PA RN License (Required)
1-2 years of experience in an acute care or geriatric setting required.
Knowledge of Long-Term Care Admission Policies/Procedures and Medicare/Medicaid regulations.
Knowledge of Infection Control Polices/ Procedures as set forth by OSHA and the CDC guidelines.
Knowledge and experience with third party insurance required
Data entry experience required.
